ELITE FUTSAL WINTER PROGRAM - PAY BALANCE

$282.50

To remain in good standing, the outstanding balance for the program must be received on or before January 22nd. As a curtesy, if the full balance of $282.50 is paid by this date, no late fee will be assessed. When you registered your player you agreed to the following terms (see details below).

*Payment Plan Options:

Train & Game $565: Pay $141.25 for 4 months (example: Oct 25th, Nov 25th, Dec 25th, and Jan 25th). Payment will be processed on the date of the first purchase and then monthly for 3 more months.

Late Payment Penalty:

If payments are unable to be processed on the monthly due dates (according to first payment) then a late penalty of $35 will be assessed for each due date missed. Extraneous circumstances will be taken into consideration but must be discussed with the administrative staff in a timely manner (within 3 days of any missed payment).
